Here's how Live Seminars work...
Registration and Confirmation for Live Seminars
Enrollment is confirmed on receipt of registration and payment, or a space can be held with 15% nonrefundable deposit of full tuition by check or credit card. If you have not received confirmation two weeks after mailing your registration, please call our office at 715-549-5551 to confirm your registration. We recommend that you contact us by phone as soon as you have been approved by your company, so that we can hold your spot with a credit card. Credit card holds are subject to a 15% administrative fee if cancelled. Late registration will be accepted contingent on space availability. |

Enrollment and Deadlines for Live Seminars
Enrollment is limited. Early registration is encouraged to guarantee space and prevent cancellation due to inadequate response. The early bird discounts apply when registration and payment are received or postmarked by the early bird expiration date. Participants are accepted on a first-come, first-served basis. Payment in full is due two weeks prior to date of course. Late registration will be accepted contingent on available space. |

Cancellations and Refunds for Live Seminars
Registration fee less a 15% administrative charge is refundable if written cancellation is received at least one week prior to program date. No refunds will be given after that time. Keep Pace reserves the right to cancel at any time with full refund. It is recommended that you call the Keep Pace office (715-549-5551) before making airline reservations to ensure the class is not cancelled due to inadequate response. |

Approved Provider Status/CEU's for Live Seminars
Keep Pace is an Approved Provider by the AOTA. All LIVE SEMINARS are submitted to the MN Board of Physical Therapy. Courses are eligible in other states for contact hours.

Certificates of Attendance are distributed at the conclusion of the course reflecting the actual number of hours of attendance. |

Travel Information for Live Seminars
Once you have enrolled, you will be sent detailed driving directions to the seminar site. We also recommend that you check the published websites of the seminar site and/or hotel for maps, etc. It is recommended that you call the Keep Pace office before making airline reservations to ensure the class is not cancelled due to
inadequate response. |

Here's how Video Rentals work...
- DVD Rentals are for 30 days, not including shipping
time.
- View the entire program at your own pace. When you are finished,
complete the Verification Exam and Course Evaluation. Return the
videos, exam and evaluation. You will receive your Certificate
of Contact Hours in the mail.
- Prior to mailing back the program, we recommend that you fax
your completed Verification Exam to our office to assure successful
completion. If the Verification Exam is not successfully completed,
the viewer may retake the exam.
- The Certificate of Contact Hours will be dated and mailed on
the day the videos and required forms are received at the Keep
Pace office.
- For DVD videos a prepaid USPS return shipment label is provided for your convenience,
so the rental program can be dropped off at any US Post Office.
- Handouts may not be reproduced under any circumstances. They
are copyright protected.
- No agency, other than Keep Pace, is authorized to provide a
substitute Certificate of Contact Hours or in-service certificate
of training based on these video seminar rentals.
- No agency is authorized to charge admission to anyone who views
the rental seminar.
- RENTAL FEE IS FOR A 30-DAY RENTAL PERIOD. A 30-day extension
can be obtained if there are 2 or more additional viewers.
- Purchase to own: DVD's can be purchased to own for an additional $50.00.

CEU Information for Video Rentals
FOR VIDEOS, it is the viewer's responsibility to check
with their state licensure board for confirmation and acceptance
of Video Long Distance Learning.

Keep Pace is an Approved Provider for AOTA CEU's for Long Distance
Learning. The assignment of AOTA CEU's does not imply endorsement
of specific course content, products, or clinical procedures by
AOTA.
Physical Therapy: All video seminars are submitted to the Minnesota Board of Physical Therapy. They are eligible in other states for contact hours. If you are a PT practicing outside of MN, you will need to check with your state licensing agency as to their acceptance/procedure for submittal and approval of long distance learning.

Group Discounts for Video Rentals
- A 50% discount applies for each additional viewer. The first
viewer pays full price and each additional viewer receives 50%
off.
- ONE SET OF VIDEOS will be sent with multiple manuals.
- After a video has been mailed to a designated group, additional
viewers may be added later at the half price rate. However, each additional later shipment of manuals will incur a $10 Shipping and Handling
fee.
Additional viewers may complete the Keep Pace
Registration form on our Online Registration page or call our office at 715-549-5551 to register.
Specify the primary viewer/facility of the Video Seminar.
Since subsequent Shipments of seminar manuals and materials incur
a $10 S&H fee, we recommend that people register for additional
materials as a group if possible. The S&H fee is per shipment, not
per individual. |

Cancellations and Refunds for Video Rentals
A 15% cancellation fee may apply if cancellation occurs after
a video is ordered but not yet mailed.
There are no cancellations or refunds AFTER the video has been ordered
and mailed to the viewer(s). |

Certificates
of Contact Hours for Video Rentals
Viewers will receive a Certificate of Contact Hours upon return
of the videos and successful completion of the Verification Exam
achieving a grade of 70% or better. |
